Central Valley hospitals write off $154 million for charity care

Hospitals in California’s Central Valley wrote off a total of $154 million in charity care last year, according to an updated list published by The Business Journal on November 14, 2025. The list ranks hospitals in Fresno, Kings, Madera, and Tulare counties based on the amount of patient medical bills written off as charity care between January 1 and December 31, 2024.

Community Regional Medical Center (CMC) in Fresno led the rankings. The hospital provided $94.07 million in charity care and reported $64.9 million in unpaid medical bills for 2024. During this period, CMC discharged 547 indigent inpatients and had 27,165 outpatient visits where patients were unable to pay their bills out of a total of 307,008 outpatient visits.

Clovis Community Medical Center was second on the list with $20.79 million provided in charity care and $26.27 million reported as unpaid patient medical bills last year. Of its 38,107 discharged inpatients, 65 were identified as indigent.

Kaweah Health Medical Center in Visalia ranked third with $17.15 million given in charity care during 2024. The center discharged 23,885 inpatients that year; among them, 150 were indigent patients. Out of a total of 610,263 outpatient visits at Kaweah Health, financial assistance was required for 4,398 patients.

Saint Agnes Medical Center (SAMC) in Fresno moved up to fourth place from fifth on the previous list. SAMC provided $11.83 million in charity care and reported $5.62 million in unpaid medical bills for the year. The center discharged 33 indigent patients—representing about 0.1% of its total discharges—and saw a total of 170,429 outpatients; among them, financial assistance was needed for 248 individuals.

Kaiser Permanente Fresno completed the top five with $10.33 million provided as charity care at its Fresno location during the same period and reported $56,039 in unpaid medical bills.

Hospital staff encourage families and individuals who are unable to afford medical treatment to apply for financial assistance programs available at these facilities.
